Key Responsibilities
Assessing patients’ medical and mental health needs
Providing a plan of care that proactively manages conditions
Ensuring compliance with local, state, and federal agencies
Prescribing medications and ordering lab work
Monitoring patients’ compliance and modifying treatment plans
Working with RN case managers as part of an integrated care team
Required Qualifications
FNP, AGNP, AHACNP or PA license
Degree from an accredited APRN or PA program
3 years’ experience in clinical nursing or rehab in geriatric populations
Electronic Health Records experience
Working knowledge of Microsoft applications
Preferred Qualifications
Primary Care in Geriatric Medicine
3 years of Long Term Care experience
