Key Responsibilities
Engage members face-to-face and/or telephonically to assess needs
Develop and implement person-centered care plans
Partner with internal care team and community resources
Provide education and coaching for member self-management
Support proactive discharge planning and care transitions
Advocate for members and families
Required Qualifications
1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
1+ years of experience with MS Office (Word, Excel, Outlook)
Access to reliable transportation
Ability to travel within assigned territory
Preferred Qualifications
Bachelor’s degree in Psychology, Special Education, or Counseling
