Join Molina Healthcare as an IRIS Consultant, where you'll partner with individuals in the Wisconsin IRIS Program, helping them achieve their long-term care goals. This remote position allows you to make a significant difference in the lives of people with disabilities and older adults by providing guidance, support, and community resources.
Key Responsibilities
Build relationships with IRIS participants and help them navigate the Wisconsin IRIS program
Conduct a minimum of four in-person meetings with participants annually
Document orientation and planning activities within the IRIS data system
Assist participants in identifying and achieving personal outcomes within their IRIS budget
Educate participants on budget management and resources
Collaborate with other staff to ensure successful implementation of plans
Attend monthly team meetings and participate in weekly check-ins
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field
1+ year of direct experience in social services for individuals with disabilities and older adults
Ability to work independently and be self-motivated
Knowledge of Long-Term Care programs
Excellent problem-solving and critical thinking skills
Strong time management and prioritization skills
Ability to maintain professional relationships
Resourceful with knowledge of community resources
Proficient in technology tools including Outlook, Skype, Teams, and Office applications
Valid driver's license and good driving record
