Travel Physical Therapist (PT) – Long Term Care (LTC)
Location: Buffalo, MO
Start Date: 11/3/2025 | Duration: 13 Weeks
Shift: Days | 8-hour shifts | Monday–Friday | 35 hours guaranteed
Start Window: Flexible between 6:00 AM – 10:00 AM (must be consistent and communicated with DOR)
Weekends: None | On Call: None

Position Overview

HumanEdge is seeking a dedicated Travel Physical Therapist (PT) to join a Long Term Care (LTC) team in Buffalo, Missouri. The PT will evaluate, treat, and rehabilitate patients with physical impairments to restore mobility, function, and independence using evidence-based therapy practices.

Responsibilities

• Conduct patient evaluations and screenings to establish individualized plans of care

• Provide therapeutic treatments including exercises, ambulation training, manual therapy, modalities, and wound care

• Use standardized assessments and evidence-based interventions

• Adjust treatment plans and goals based on patient progress

• Collaborate with interdisciplinary healthcare teams

• Educate patients, caregivers, and families on therapy goals and safety

• Document treatment and progress in EMR per regulatory guidelines

• Participate in team meetings and care conferences

• Supervise PTAs and rehab aides as appropriate

• Adhere to APTA standards, state practice guidelines, and facility policies

Requirements

Required:

• Active Missouri PT license

• 1 year of PT experience

• Long Term Care (LTC) or Rehab experience

• Strong clinical evaluation and documentation skills

Acceptable:

• New grads with Level II clinical rotation experience (not observation-only)
