Key Responsibilities
Evaluates and treats patients in accordance with standards of care
Monitors care quality using available data
Coordinates patient services including specialty referrals
Participates in daily huddles with the care team
Completes medical record documentation timely
Participates in quality of care meetings
Assures compliance with licensing and accreditation
Spends 100% of time clinically focused on direct patient care
Required Qualifications
Current and unrestricted medical license or willing to obtain
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
High level of skill with interpersonal relationships
Engaged in integrated team-based care model
Willingness to learn/adapt to value-based care
Superior patient/customer service skills
Basic computer skills including email and EMR
Preferred Qualifications
Board Certification in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
2-5 years applicable experience preferred
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines and coverage
Bilingual skills
Knowledge of HEDIS quality indicators
