Key Responsibilities
Coordinate complex discharges and develop plans for safe transitions
Manage patient cases and engage patients and families
Focus on medication reconciliation and reducing avoidable readmissions
Provide consistent transition overview across different care settings
Required Qualifications
Oregon Registered Nurse License
3 years Acute care hospital experience
Preferred Qualifications
Bachelor's Degree in Nursing
1 year home health, mental health, substance use, hospice, and/or utilization review
Progressive nursing leadership experience, including charge nurse role
