Job Description
Join our compassionate team as a Hospice Spiritual Care Counselor, where you will provide essential spiritual support to patients and families in a collaborative environment. With a focus on individualized care, you will play a key role in enhancing the quality of life for those facing end-of-life challenges.
Key Responsibilities
Ensure spiritual assessment of patients and families/caregivers in the hospice program
Facilitate the development of individualized plans of care
Provide direct spiritual care to patients and families/caregivers
Serve as liaison and support to community chaplains and spiritual care counselors
Provide consultation, education, and support to the interdisciplinary group on spiritual care
Provide for funeral or memorial services for patients as requested
Plan periodic memorial services for personnel, volunteers, and community clergy/spiritual counselors
Actively participate with quality assessment performance improvement teams and activities
Required Qualifications
Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
Experience working with death and dying, individuals/family/caregiver
Preferred Qualifications
