Job Description

Join CenterWell Home Health as an RN Clinical Manager to lead a dedicated team in providing quality home health care services. Your leadership will ensure effective clinical operations while making a significant impact on patient care and outcomes.

Key Responsibilities

  • Coordinates and oversees all direct care patient services provided by clinical personnel
  • Develops, plans, implements, analyzes, and organizes clinical operations
  • Conducts/delegates the assessment and reassessment of patients
  • Manages the assignment of caregivers
  • Responsible for and oversees the delivery of care to all patients
  • Instructs and guides clinicians to promote effective performance
  • Monitors cases to ensure compliance with regulatory agencies
  • Coordinates communication between team members and caregivers
  • Participates in sales and marketing initiatives
  • Supervises all clinical employees assigned to a specific location
  • Handles employee corrective action and discipline issues
  • Participates in the interviewing, hiring, training, and development of direct care clinicians
  • Analyzes situations and identifies problems
  • Responsible for review of clinical staff documentation
  • Responsible for QA/PI activities
  • Provides direct patient care in emergencies
  • Acts as Branch Director in his/her absence
  • Maintains relationships with referral/community sources

Required Qualifications

  • Graduate of an accredited School of Nursing
  • BSN required (NY state requirement)
  • Current state license as a Registered Nurse
  • Proof of current CPR
  • Valid driver’s license, auto insurance and reliable transportation
  • Two years as a Registered Nurse with at least one-year of management experience in a home health environment
  • Certified Home health experience is required (NY state requirement) or four years' experience in home care and six credit hours, or the equivalent, of education/training in public health and principles of management
  • OASIS experience, required

Preferred Qualifications

  • Management and people leadership experience
  • OASIS certification (HCS-O, COQS, and/or COS-C)
  • Homecare Homebase (HCHB) experience
  • CMS PDGM billing knowledge and/or experience
