Credentialing: Active Indiana license or IMLC accepted
Board Status: Board Certified or Board Eligible (must certify within 5 years of residency)
Certifications: ACLS (if conscious sedation privileges requested), BLS required
Vaccinations: COVID and Flu vaccines required (or valid exemption)
