Key Responsibilities

• Provides direct patient care using the nursing process
• Performs plan of care interventions, including medication administration and clinical treatments
• Documents patient care given
• Monitors and communicates patient condition to care team
• Assists patients with activities of daily living
• Collaborates across disciplines to coordinate patient care
• Evaluates learning needs and provides patient education
• Acts as an advocate for patient well-being
• May perform duties of CNA, MA, or LPN as needed
• Participates in staff meetings and adheres to policies
