Join Kaiser Permanente as a Patient Care Coordinator, where you'll oversee the management and coordination of care for acute inpatient populations, ensuring effective transitions and preventing readmissions. This role is critical in collaborating with healthcare teams to enhance patient outcomes.
Key Responsibilities
Complete initial face-to-face assessments for admitted members
Document assessment outcomes and communicate with healthcare team
Participate in daily Care Without Delay rounds
Review and document discharge plans
Manage care progression with physician and nursing staff
Collaborate with Social Worker for long-term care coordination
Send referrals for post-acute levels of care
Identify and escalate delays in care
Schedule follow-up appointments for high-risk patients
Participate in Quality Assurance duties
Required Qualifications
Minimum of one (1) year in an acute medical/surgical/ED or critical care nursing area
Associate Nursing degree required
Registered Nurse License (Virginia) within 6 months of hire AND Registered Nurse License (Maryland) within 6 months of hire OR Compact License: Registered Nurse within 6 months of hire
Basic Life Support certification
Experience using an electronic medical record system
Effective communication skills with physicians, members, and hospital staff
Ability to work independently and apply critical thinking skills
Excellent time management skills
Preferred Qualifications
Recent acute care, case management, or home health experience
