Job Description:
We are seeking experienced RN Care Managers to join our team for a 13-week assignment. The RN Care Manager serves as the primary link between patients, healthcare teams, the organization, and payors, coordinating concurrent utilization management activities and ensuring smooth patient transitions across care settings. This role requires strong clinical expertise, excellent communication, and the ability to collaborate with interdisciplinary teams and community resources to optimize patient care.

Key Responsibilities:

  • Assess patient needs and coordinate necessary resources for transition to the next care setting.
  • Lead and educate interdisciplinary teams during daily care rounds, care conferences, and care planning sessions.
  • Communicate effectively using written, verbal, and electronic systems.
  • Coordinate with community organizations for durable medical equipment, insurance authorizations, and transportation.
  • Monitor length of stay and manage care for inpatient hospital populations.
  • Maintain knowledge of hospital reimbursement methods and quality measures relevant to care management.

Qualifications & Skills:

  • Associate or Bachelor’s degree-prepared RN with 3–5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or utilization review.
  • Experience in utilization review and discharge planning required.
  • Certification by a nationally recognized case management or utilization review organization preferred; CCM or ACM preferred.
  • Active BLS certification required.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work effectively with patients, families, interdisciplinary teams, and community resources.
