Key Responsibilities

• Provides direct patient care using the nursing process
• Performs plan of care interventions, including medication administration
• Documents patient care given
• Monitors and communicates patient condition
• Assists patients with daily living activities
• Collaborates across disciplines to coordinate patient care
• Evaluates learning needs and provides education to patients/families
• Acts as an advocate for patient well-being
• May perform duties of CNA, MA, or LPN as needed
• Attends staff meetings and participates in projects
