Job Description

Join Optum as a Nurse Practitioner or Physician Assistant, where you'll provide vital care assessments for patients transitioning from rehab to home. This part-time role offers autonomy and the opportunity to make a significant impact on patients' health journeys.

Key Responsibilities

  • Perform comprehensive assessments and document findings
  • Ensure accurate ICD10 coding and documentation
  • Address all quality elements and documentation
  • Conduct initial medication reviews
  • Communicate with patients and team members regarding medical concerns
  • Participate in SCC quality initiatives

Required Qualifications

  • Certified Nurse Practitioner through a national board
  • Active and unrestricted APC license in the State of Connecticut
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire
  • Access to reliable transportation
  • Availability to work 10 hours per week
  • Ability to gain a collaborative practice agreement, if applicable

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Ability to lift a 30-pound bag and navigate stairs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills with medical records
  • Ability to develop and maintain positive customer relationships
  • Adaptability to change
