Job Description
Join Sunshine State Health Plan as a Licensed Practical Nurse (LPN) to make a significant impact on the health of our communities. You will develop personalized care plans, educate members and their families, and ensure they receive high-quality, cost-effective healthcare.
Key Responsibilities
Develop and facilitate complex care management activities
Create personalized care plans and educate members and families
Evaluate member needs and barriers to care
Coordinate between members, caregivers, and care provider teams
Monitor member status and update care plans as necessary
Provide resource support for local services
Conduct assessments through various outreach methods
Document and maintain compliance with regulations
Educate members on healthcare options and benefits
Provide feedback for improving care delivery
Required Qualifications
Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
2 – 4 years of related experience
Registered Nurse - State Licensure and/or Compact State Licensure
Preferred Qualifications
Bilingual Spanish speaking
Minimum of two years of pediatric experience
