Key Responsibilities
Develop and facilitate complex care management activities for mental and behavioral health members
Evaluate member needs via phone or in-home visits
Develop ongoing care plans for high acuity members
Coordinate with community resources and care provider teams
Monitor and document member information and care management activities
Provide education to members and families on healthcare options
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2-4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Experience with pediatric care
