Duties and Responsibilities:

• Provide compassionate end-of-life care to patients in homes, hospitals, or hospice facilities.
• Conduct comprehensive patient assessments, including physical, emotional, and spiritual needs.
• Develop, implement, and adjust individualized care plans focused on comfort and quality of life.
• Administer medications for pain management, symptom control, and comfort care.
• Educate patients and families about the disease process, treatment options, and what to expect during end-of-life care.
• Offer emotional support and counseling to patients and their families.
• Collaborate with interdisciplinary team members (physicians, social workers, chaplains, CNAs, therapists) to ensure holistic care.
