Job Title: Home Health Registered Nurse – Days

Employment Type: Contract - W2

Location: Lombard, IL
Coverage area: South side of Chicago (south of downtown)

Facility Details:

• Facility size: Varies (home-based care)
• EMR: Must have HCHB (Homecare Homebase) / OASIS experience

• Scrub color: Any color acceptable

Start Date: ASAP

Contract Details:

• 13-week contract
• 40 hours per week (expected to work full 40 hours weekly)
• Weekend requirements indicated on requisition
• No on-call requirement
• No OT allowed
• Must be flexible with floating as needed
• RTO: Must be approved in advance; any time off must be made up
• Extension opportunities possible

Licensure & Certifications:

• Active RN license required
• BLS certification required

Experience Requirements:

• Minimum 2 years of experience required
• Must have recent home health nursing experience
• Must have HCHB and OASIS documentation experience (OASIS E experience required after Jan 2023)
• No more than a 1-year gap in employment
• Must meet productivity standards (average 26–28 points; typically 5–7 points per day)
• Approximately 20–25 cases per week (could mean 3 to 7 patients per day depending on point values)
• Absolute must: true home health experience with strong HCHB/OASIS skills
• All documentation must be completed same day at the patient's home

Territory & Travel:

• Candidates must live within 30 minutes of their territory
• If traveling, must secure housing nearby
• Mileage reimbursement available
• Once a week, mandatory in-person office day (usually Tue, Wed, or Thur)

• Approximately 2–3 days, including online modules and team introduction
