Medical Solutions Allied is seeking a travel Physical Therapist for a travel job in Louisville, Kentucky. COVID and flu vax required30 GH, up to 40 hoursRates; overtime 1.Mileage and Travel Time: Facility agrees to pay forty five (45) cents per mile for any miles driven by Personnel between Facility's sites as assigned by Facility. Mileage only applies to distance driven in addition to the normal daily commute to the initial work site. Facility shall also be billed for Personnel travel time between assigned sites.Cancellation Policy: Terminationof Personnel with less than two weeks written notice will incur the rate for Personnel's documented work hours up to two weeks. If documented work hours are less than thirty (30) during notice period, it is by choice of Company / Personnel and will not be paid by Facility.Overtime: Facility agrees to pay 1 ½ times the applicable bill rate for all overtime hours worked by Personnel. Overtime shall be all hours worked by Personnel in excess of forty (40) hours in a normally scheduled workweek.24 2ALL INVOICES SHOULD BE EMAILED TO Accounts Payable,p: | f:Overtime hours of Personnel must receive Facility's prior approval. Failure to receive prior approval will result in standard hourly rate and not overtime rate.Holidays: regular pay for holidays and weekendFees: Should a traveler want to become permanent, a completed 13-week travel assignment will be considered payment in full toward placement fees. Facility will not pay Permanent Placement fees on any Personnel that have completed a 13-week travel assignment. If Personnel want to become permanent prior to completing a 13-week assignment, Facility will agree to pay Company a $5000 placement fee for PT/OT/SLP and $2500 placement fee for PTA/OTA.
