Key Responsibilities
Provide ongoing care-coordination and case management services
Guide linkage to appropriate clinical services including medication-assisted treatment, behavioral health treatment, and social services
Foster supportive relationships with individuals with Substance Use Disorder
Ensure continuity of care by meeting patients in various clinical settings
Identify health and wellness needs and barriers for patients
Perform outreach and act as advocates for patient services
Attend clinical and programmatic conferences as required
Engage in ongoing professional development and training
Required Qualifications
Bachelor's degree in a related field with 1 year of relevant experience
Associates degree with 2 years of relevant experience
Certified Peer Recovery Specialist with 2 years of relevant experience
Certified Medical Assistant with 4 years of relevant experience
Valid driver’s license
Own vehicle or reliable transportation
