Key Responsibilities
Use nursing expertise to connect mobile healthcare resources
Conduct telephonic patient assessments
Provide health education and support healthy behaviors
Engage in telemonitoring activities
Participate in team care planning and mentoring
Represent GMR’s innovative programs in the community
Facilitate and educate for GMR’s integrated healthcare training
Manage time, resources, and documentation efficiently
Required Qualifications
Valid RN Compact State License
Minimum of 3+ years of RN experience in Emergency, Acute Care, or a similar fast-paced environment
Experience with clinical data collection, ethical principles, and patient advocacy
Graduate of an accredited nursing program
Current RN license in a compact state
Current CPR certification
Preferred Qualifications
Bachelor’s or master’s degree in healthcare administration, Public Health, or related fields focused on community health or disease education
