Physical Therapist (PT) – Home Health

MedAdventures is looking for a compassionate Physical Therapist (PT) to join our Home Health team in York, PA. In this role, you’ll provide one-on-one care in patients’ homes, helping them regain strength, mobility, and independence—all while working in a flexible and supportive environment.

Whether you’re experienced or newer to home health, we provide training to get you comfortable with our systems and processes. You’ll have the autonomy to manage your patients while still having a team ready to support you.

What You’ll Do:

• Conduct thorough in-home PT evaluations and develop individualized care plans
• Provide hands-on therapy to help patients improve mobility, balance, and strength
• Educate patients and caregivers on home exercises, safety, and fall prevention
• Accurately document visits and progress using OASIS and Homecare Homebase (HCHB)
• Collaborate with nurses, therapists, and case managers to ensure coordinated, high-quality care

What We’re Looking For:

• Active Pennsylvania PT license (or eligibility to obtain)
• Experience with OASIS and Homecare Homebase (HCHB) preferred, but training is provided
• Prior home health experience is a plus, but not required
• Strong communication, clinical reasoning, and organizational skills
• Dedication to patient-centered care
