Job Description

Join Monte Nido & Affiliates as a Hybrid Social Worker, where you'll play a vital role in care coordination and case management for clients in various treatment programs. Key Responsibilities

  • Care Coordination & Case Management: Partner with clinical team for case management and care coordination.
  • Community & Family Support: Connect clients and families with community-based programs and resources.
  • Referral Management: Manage referrals to community programs and facilitate collaboration.
  • Transitions Between Care Levels: Ensure smooth transitions between levels of care.
  • Prepare Clients & Families for Transition: Provide support and information for transitions.
  • Ongoing Resource Connection: Connect clients with resources post-discharge.

Required Qualifications

  • Master’s degree in Social Work or related field
  • Experience with CareOregon/Medicaid
  • Proven experience in case management or care coordination
  • Strong communication skills
  • Ability to work collaboratively with a multidisciplinary team
  • Understanding of community resources and insurance processes

Preferred Qualifications

  • LCSW preferred
