Job Description
Join Sunshine State Health Plan as a Senior Care Navigator - Pediatric, where you'll assess and coordinate care management activities to improve healthcare access for members. Utilize your skills to develop personalized care plans and collaborate with healthcare providers to ensure quality care outcomes.
Key Responsibilities
Assess, plan, implement, and coordinate care management activities
Develop personalized care plans/service plans
Coordinate services between members and care provider team
Monitor progress towards care plan goals
Provide psychosocial and resource support
Identify barriers for care coordination
Collaborate with healthcare partners
Document member information and care management activities
Perform on-site visits
Educate care managers and members on procedures and benefits
Serve as a Subject Matter Expert (SME)
Required Qualifications
Bachelor’s degree
4 - 6 years of related experience
Current state’s clinical license (preferred)
Pediatric experience or Master's degree in social work with pediatric care experience
Preferred Qualifications
Graduate from an Accredited School of Nursing (if holding clinical licensure)
Ability to travel up to 80% locally
