Job Description
Join Christian Living Communities as an MDS Coordinator, where you'll play a crucial role in developing and coordinating resident assessments and care plans. Your expertise will help ensure quality care and compliance with state regulations, making a positive impact on residents' lives.
Key Responsibilities
Oversees and evaluates the gathering of information for assessments and care plans
Monitors and guides the completion of PPS, OBRA assessments, Triggers, and RAPs
Maintains and updates written policies and procedures for resident assessment and care plan
Transmits completed MDS to appropriate parties weekly
Assesses prospective residents in conjunction with team members
Performs nursing functions when needed
Completes administrative duties such as medical forms and reports
Develops and maintains a quality assurance program for assessments and care plans
Participates in on-site surveys by authorized government agencies
Required Qualifications
Registered Nurse with a current, unencumbered license in the state of operation
At least two years of managerial experience in a hospital or nursing care
Knowledge and skills to provide care appropriate to age-related needs
Schedule flexibility with ability to be rotating managerial on-call
Ability to problem solve independently
Basic knowledge of office software applications including Word, Outlook, and Excel
Excellent customer service skills
