Job Description
Join The Jewish Board as a Case Manager, where you'll support adults with serious mental illness in achieving independence and healthy living through personalized care and advocacy. You'll play a pivotal role in guiding residents on their recovery journey, facilitating essential services, and ensuring their voices are heard within the community.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning with residents
Completes regular and timely documentation of all resident interactions
Supports residents in meeting individual goals and medication management independence
Advocates on behalf of residents with other service providers
Facilitates monthly roommate meetings and runs resident support groups
Participates in planning and execution of special and holiday events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
