Assignment Details:

  • Duration: Up to 32 weeks
  • Rates:
    • Travel: $100/hour (51-100 miles radius)
    • Local: $90/hour (50 miles or less)
    • Radius measured by straight line, not driving distance
  • Schedule:
    • Monday to Sunday, 8 AM - 5 PM
    • Weekend rotations: 1 Saturday/Sunday per month with 2 compensatory days during the week
    • No block schedules permitted
    • Self-scheduling allowed within department guidelines
    • No call requirements
    • Holiday requirements: As needed
  • Orientation:
    • Bi-weekly, next session: August 18
    • Duration: 2-3 days
    • Includes Netsmart documentation system training
  • Workload:
    • 5-6 clients per day
    • Average caseload: 18-22 clients for case managers
    • Mileage typically limited to a 30-mile radius unless emergent need

Candidate Requirements:

  • Licensure: Active PT license (please note any additional licenses)
  • Experience:
    • Minimum 3 years of clinical experience in home health required
    • Prior travel experience required; 2 previous travel assignments preferred
  • Skills:
    • Strong clinical and assessment skills
    • Fluency in navigating various Electronic Medical Records (EMRs)
  • Certifications:
    • Required: BLS, OASIS experience
    • Preferred: OASIS Nurse certification
  • Submission Requirements:
    • Complete work history (bed count, EMR, facility type, etc.)
    • Explanation of any employment gaps over 30 days
    • Covid and Flu declinations accepted (proof or declination required)

  • Unit: Adult Home Health
  • Specialty: Home Health
  • Patient Population: Cardiac, pulmonary, diabetes, wound care, post-surgery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson’s, ostomy, catheters, MVA, etc.
  • Age Range: 30-100
  • Patient Ratio: 5 visits per day equivalent
  • Documentation System: Netsmart
  • Dress Code: Business casual or any color scrubs, closed non-skid shoes, minimal jewelry, groomed hair/beard, minimal nail polish
  • Special Equipment: Provided (laptop, iPhone, trunk bin/visit bag with VS needs, wound care, catheters, etc.)
  • Unit Procedures: Phlebotomy, catheters, IV, coag-u-check, port access, wound vacs, TPN, feeding tubes, drains, nebulizer
  • Support: Respiratory available 24/7; no anesthesia or ventilator management required
  • Coverage Area: Chittenden County and Grand Isle County

Additional Notes:

  • Transportation: Public transportation not accessible; reliable personal or rental vehicle strongly recommended
    • Vehicles must have all-season or snow tires (November-April)
  • Floating/Charge: Not required
  • IVS to Offer: Yes
  • Teaching Facility/Trauma Level: Not applicable (post-acute care)
