Join MJHS as an RN Case Manager and play a vital role in hospice and palliative care, providing essential support to patients and their families. In this fee-for-service position, you will conduct assessments, develop care plans, and collaborate with healthcare teams to ensure quality patient care.
Key Responsibilities
Perform physical and psychosocial assessments
Electronically document visit occurrences
Develop and implement nursing plans of care
Contact patient's physician for care plan changes
Supervise, train, and evaluate home health aides and homemakers
Required Qualifications
Associates/Diploma from an accredited school of Nursing
New York State RN licensure
Valid NYS driver's license
Preferred Qualifications
BSN preferred
At least two years experience in medical-surgical, community health, and/or oncology nursing
Proficiency with Word and Excel
Experience working within an interdisciplinary team environment
Effective communication, listening and teaching skills
