Job Description
Join The Jewish Board as a Case Associate, where you will make a significant impact by supporting client care in the community. Work collaboratively to develop action plans and utilize resources effectively, while benefiting from comprehensive support and a commitment to professional growth.
Key Responsibilities
Function as a case planner and supply appropriate interventions
Develop action plans to effectively identify and utilize existing resources
Conduct community contacts on behalf of social worker
Evaluate clients' living conditions through home visits
Provide structure to clients and evaluate independent living skills
Run groups under the supervision of a social worker
Evaluate clients for individualized treatment programs
Facilitate client referrals to service providers
Perform individual/group counseling based on client needs
Identify help for clients in obtaining needed services
Provide case management and necessary documentation
Required Qualifications
BA Degree in a relevant field
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual in Haitian-Creole
