Job Description
Join CenterWell Home Health as a Licensed Vocational Nurse and make a meaningful impact by providing personalized care that helps patients regain independence in their own homes. Work collaboratively with a dedicated team to develop and manage care plans that support patient recovery.
Key Responsibilities
Provide professional nursing services under the direction of a RN
Provide skilled nursing interventions in the treatment of patient/client illness
Apply knowledge and skills in accordance with accepted standards of clinical practice
Confer with the RN Supervisor regarding needed changes in the Plan of Treatment
Maintain contact with patients, physicians, and other members of the healthcare team
Prepare, submit and maintain documentation as required
Required Qualifications
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
Current nursing license in the practicing state
Valid driver's license, auto insurance, and reliable transportation
Current CPR certification
Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
