Job Description
Join The Jewish Board as a Case Manager, where you'll play a vital role in supporting adults with serious mental illness. This position involves guiding residents towards independence, helping them develop essential living skills, and advocating for their needs in a compassionate, culturally competent environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Supports residents in achieving independent living goals
Advocates on behalf of residents with service providers
Completes documentation of resident interactions
Participates in staff meetings and trainings
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
