Job Description
Join Beacon Brook Center for Health & Rehabilitation as an MDS Director (RN) to lead MDS assessments and ensure compliance with state and federal regulations. Make a meaningful impact on resident care while enjoying professional growth in a supportive environment.
Key Responsibilities
Manage the Minimum Data Set (MDS) assessments for resident care plans
Ensure compliance and accuracy in assessments
Determine Patient Driven Payment Method (PDPM) for potential admissions
Monitor Case Mix Index (CMI) scores for potential risks affecting reimbursement
Train and mentor MDS Coordinators
Coordinate comprehensive resident assessments and care planning
Collaborate with interdisciplinary teams for accurate data collection
Provide ongoing education to staff and leaders
Track and analyze assessment data for compliance and optimization opportunities
Drive a culture of continuous improvement in nursing care
Required Qualifications
Valid state RN license preferred
Direct care in a long-term care setting and/or MDS Coordinator experience required
Proficient in state and federal regulations governing the MDS and billing process
Knowledge of Electronic Medical Record (EMR), PDP, MDS 3.0, Medicaid and Medicare requirements
Deadline driven and detail-oriented
Excellent written and verbal communication and interpersonal abilities
Ability to work effectively in a multidisciplinary team environment

Advanced degree or certification preferred
