Home Health Occupational Therapist OT Full Time 30 Hours

Key Responsibilities

• Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function
• Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes
• Provide a written evaluation and plan of care defining occupational therapy needs, goals, progress, and expected outcomes
• Deliver occupational therapy services per prescribed physician orders
• Maintain plan of care through timely documentation and modification of goals and treatment
• Collaborate on discharge planning and educate patients and caregivers on home exercise plans
• Coordinate services with healthcare team, physicians, and guardians
• Document according to policy and procedure with minimal supervision
• Maintain effective communication with staff, healthcare team members, and family
• Supervise other personnel (OTA, HHA) as applicable
• Complete OASIS assessments where allowed by state regulations
