Key Responsibilities
Provide high quality health care through direct screening, diagnosis, and formulation/implementation of treatment plans
Document all care in patients' permanent medical records
Provide case management and consult with other department providers regarding treatment plans
Perform other duties as requested
Required Qualifications
Minimum two (2) years of clinical experience
Masters level degree or higher (M.S.W., M.S., M.A., M.Ed.)
Licensed Clinical Social Worker (Oregon) or equivalent licenses within 6 months of hire
National Provider Identifier required at hire
Preferred Qualifications
Thorough knowledge of psychosocial diagnostic categories and treatment approaches
Familiarity with Cognitive Behavioral Therapy, Dialectical Behavioral Therapy
Ability to access, diagnose and treat child and adult mental health patients
