Job Description

Key Responsibilities
Admit patients eligible for hospice services under physician's order
Assess and evaluate patient needs/problems
Report patient status to clinical leadership and physicians
Update care plans and revise patient goals as needed
Complete informational visits and obtain patient consents
Ensure the use of the 4Ms and provide Age-Friendly Care
Required Qualifications
Current unencumbered registered nurse license
Valid driver's license and good driving record
Ability to work in a field setting
Ability to assess patient needs and formulate individualized care plans
