Join UnitedHealthcare as a Bilingual Case Manager Coordinator, where you'll manage care for members with diverse medical and behavioral needs. You'll engage with members in their homes to develop tailored care plans and advocate for their health while collaborating with a dedicated care team.
Key Responsibilities
Engage members face-to-face and/or telephonically to complete comprehensive needs assessments
Develop and implement person-centered care plans
Partner with internal care team, providers, and community resources
Provide education and coaching to support member self-management
Support proactive discharge planning and manage Care Transition
Advocate for members and families to ensure their needs are represented
Required Qualifications
1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
1+ years of experience with MS Office, including Word, Excel, and Outlook
Access to reliable transportation to meet with members and providers
Preferred Qualifications
Bachelor’s degree in Psychology, Special Education, or Counseling
