Key Responsibilities
Provide advocacy, crisis intervention, and collaborative care
Assist individuals and families with social, emotional, and economic concerns
Coordinate healthcare programs among patients, families, and healthcare teams
Respond to suspected abuse, neglect, or violence
Develop appropriate plans of care for patients and families
Participate in care conferences and transitional rounding
Required Qualifications
Bachelor’s degree in Social Work from an accredited curriculum
Licensed Social Worker (LSW) in Minnesota
Graduate from a nationally accredited nursing program (for RN role)
Bachelor’s Degree in nursing or completion of Bachelor’s in Nursing within 18 months of hire (for RN role)
Currently holds an unencumbered RN license with the State Board of Nursing (for RN role)
