Occupational Therapist (OT) – PACE Program
Location: Loveland, CO

MedAdventures is seeking a compassionate and experienced Occupational Therapist (OT) to join a PACE (Program of All-Inclusive Care for the Elderly) team in Loveland, CO . This is a rewarding opportunity to support older adults in maintaining independence, safety, and quality of life through meaningful, individualized care—both in the clinic and at home.

You’ll be part of a collaborative, interdisciplinary team delivering community-based therapy to help participants thrive in the settings they call home.

Responsibilities:

• Evaluate and treat PACE participants to improve ADLs, functional mobility, and overall independence
• Develop and adjust individualized care plans to meet participants’ needs and goals
• Collaborate with physicians, nurses, PTs, and the interdisciplinary care team
• Educate participants and caregivers on home safety, adaptive equipment, and therapeutic strategies
• Document care accurately and efficiently in accordance with program standards

Qualifications:

• Active Colorado OT license (or eligibility to obtain)
• Experience with older adult populations or in a PACE setting preferred
• Strong clinical reasoning and communication skills
• Ability to work effectively within a collaborative, team-based model
• Commitment to delivering patient-centered care in both home and clinic environments
