Job Description & Requirements

PHYSICAL THERAPIST (PT)- Full-time Travel (PT) Physical Therapist or local contract Physical Therapist (PT) needed to provide Home Health visits in a Senior Living setting in Alexandria, VA. Prior home health experience required.

$2100- $2300 net per week.

Visit weight—SOC/ROC=2 points, OASIS DC/Eval/Recert=1.5 points, Follow-up visits=1 point

Weekly expectations to meet 40 hours=28-30 points. (Approx 5 visits scheduled per day).

Qualifications:

• Determine appropriate PT, POC & D/C and make adjustments as necessary
• Master’s Degree, or higher, in Physical Therapy from an accredited physical therapy program
• Physical Therapy license, or pending, in Maryland
