Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Provide direct patient care including case management and counseling
Refer patients to appropriate levels of care and available resources
Maintain timely patient-centered documentation
Work as a team member to improve patient experience
Provide social work field supervision to second year social work students
Required Qualifications
Master’s degree in social work, Counseling, Marriage and Family Therapy or related Human Service field
Current Connecticut LCSW, LPC, LMFT, or LADC
A minimum of two years’ experience in a psychiatric setting
Highly effective verbal and written communication skills
Valid Driver's License
Preferred Qualifications
Previous experience in a healthcare setting
Cognizant of health-related social needs and cultural competence
