Key Responsibilities
Perform comprehensive evaluations of residents’ physical abilities and needs
Develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function
Provide therapeutic interventions, including exercises, manual therapy, and training on assistive devices
Monitor residents’ progress and modify therapy plans as needed to achieve goals
Collaborate with an interdisciplinary team to ensure holistic care
Educate residents and their families on therapy goals, exercises, and strategies for continued improvement
Accurately document evaluations, treatment sessions, progress notes, and discharge plans
Participate in quality improvement initiatives and contribute to the rehabilitation department's success
Required Qualifications
Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited program
Current and valid Physical Therapist license in the state of Colorado
Preferred Qualifications
Experience in skilled nursing or rehabilitation settings
Strong communication and teamwork skills with a resident-centered approach
Ability to work independently and adapt to a flexible PRN schedule
