Shift: Days, 5x8s, 8am-4:30pm

Requirements:
2yrs+ Home Health experience

• Previous Quality experience -
• COS-C (Certificate for OASIS Specialist-Clinical) Certification
• OASIS experience
• IL RN License
• BLS
• Covid vaccinated 

EMR-Matrix
Territory: Within 15 miles (may increase if census is low and they need to offer the 40 hours)
Census: 24-26 on case load. If given a bigger territory that number will go down.
Mileage: Reimbursed at the standard IRS rate.
