We are seeking a dedicated Home Health Physical Therapist to provide patient•centered care in home settings. This role is ideal for professionals passionate about home health care.

**Key Responsibilities:**

• Deliver quality physical therapy to home health patients.
• Complete documentation using HCHB and OASIS, with same•day submission.
• Manage a caseload of 20•25 cases weekly, achieving 26•28 productivity points.
• Attend weekly in•person office meetings.
• Be flexible to float and meet weekend requirements.

**Qualifications:**

• Valid PT License and BLS certification.
• 2+ years of recent home health PT experience.
• Proficient in HCHB/OASIS documentation.
• Ability to work 40 hours per week.

**Additional Information:**

• 13•week contract, potential for extension.
• Branch hours: 8 am • 5 pm, flexible for patient needs.
• Must reside within 30 minutes of the territory or secure nearby housing.
