Key Responsibilities
Assist in patient care management and care transitions
Connect patients to community resources
Educate patients about disease management and health education
Make follow-up and reminder calls to patients
Document patient interactions in EPIC
Escalate clinical questions to office nurses or Director of Clinical Practice
Collaborate with clinicians, office staff, population health team, and community partners
Required Qualifications
Experience working in a healthcare office setting or community outreach and education
Strong interpersonal skills
Ability to take direction and work independently
Preferred Qualifications
Knowledge of health promotion, nutrition, and counseling
Clinical training (i.e. MA, CNA, LPN)
Associate degree
