Job Description
Join Sunshine State Health Plan as a Licensed Practical Nurse (LPN) and play a vital role in transforming healthcare for our 28 million members. Key Responsibilities
Develop and facilitate complex care management activities
Evaluate member needs and recommend care plans
Conduct telephonic, digital, and in-home visits
Coordinate care between members, families, and providers
Monitor care plans and member outcomes
Educate members and families on healthcare options
Document member information and maintain compliance
Required Qualifications
Master's degree in Behavioral Health or Social Work
Degree from an Accredited School of Nursing
2-4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Experience in pediatric care
Behavioral health experience
