Key Responsibilities
Link adults and children to necessary services for health optimization
Develop and manage person-centered care plans
Conduct periodic assessments of members' needs
Track and arrange appointments and referrals
Provide outreach services to increase access to healthcare
Respond to member crises and inquiries
Required Qualifications
Bachelor's degree in social work, psychology, nursing, or related field
Two years of experience providing direct services to mentally disabled or chronically ill individuals
Experience working in interdisciplinary teams
Experience providing care management or care coordination in medical or behavioral health
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
