Job Description
Join our skilled nursing community as an MDS Coordinator RN in Centennial, Colorado! In this role, you'll assess residents, develop care plans, and ensure compliance with Medicare and Medicaid guidelines, making a real difference in residents' lives while working with a dedicated team.
Key Responsibilities
Gather and assess resident data to help create personalized care plans
Work closely with the care team to ensure each resident receives the support they need
Complete and submit MDS assessments on time
Stay updated with MDS regulations and best practices
Train and support staff on MDS processes
Participate in quality assurance efforts
Maintain clear and professional communication with residents and families
Collaborate with the billing team to align assessments with reimbursement requirements
Mentor new team members
Required Qualifications
Current Registered Nurse (RN) license in the state of Colorado
Minimum 2-3 years of experience as an RN in long-term care or skilled nursing community
Proven experience with MDS assessments and care planning
In-depth knowledge of Medicare and Medicaid regulations
Strong clinical assessment skills and attention to detail
Proficiency in electronic health record systems and MDS software
Excellent communication and interpersonal skills
Demonstrated ability to manage time effectively and prioritize tasks
Thorough understanding of the Resident Assessment Instrument (RAI) process
Ability to work collaboratively in a fast-paced healthcare environment
Commitment to ongoing professional development and staying current with industry trends
Preferred Qualifications
Bachelor's Degree in Nursing (BSN)
MDS Certification
