Job Description

Join Cooper University Health Care as a Patient Navigator, where you will bridge the gap between patients and clinicians, providing essential support in navigating the health care system. This role involves coordinating care and ensuring timely follow-ups while working closely with a diverse team to enhance patient experiences.

Key Responsibilities

  • Provide individual support in navigating the health care system
  • Assist with scheduling, pre-cert process, follow-up calls, and record retrieval
  • Participate in multidisciplinary rounding on inpatient oncology units
  • Act as a member of the Transition of Care team
  • Connect patients to additional services for support
  • Track patient contacts and outcomes

Required Qualifications

  • Experience in a healthcare office setting or community outreach
  • Knowledge of health care and inpatient/outpatient settings
  • Strong interpersonal skills
  • Ability to work independently and follow through with tasks

Preferred Qualifications

  • Clinical training (i.e. MA, CNA, LPN)
  • Bilingual Spanish/English
  • Knowledge of electronic health records system
