Job Description
Join Cooper University Health Care as a Patient Navigator, where you will bridge the gap between patients and clinicians, providing essential support in navigating the health care system. This role involves coordinating care and ensuring timely follow-ups while working closely with a diverse team to enhance patient experiences.
Key Responsibilities
Provide individual support in navigating the health care system
Assist with scheduling, pre-cert process, follow-up calls, and record retrieval
Participate in multidisciplinary rounding on inpatient oncology units
Act as a member of the Transition of Care team
Connect patients to additional services for support
Track patient contacts and outcomes
Required Qualifications
Experience in a healthcare office setting or community outreach
Knowledge of health care and inpatient/outpatient settings
Strong interpersonal skills
Ability to work independently and follow through with tasks
Preferred Qualifications
Clinical training (i.e. MA, CNA, LPN)
Bilingual Spanish/English
Knowledge of electronic health records system
